Risk Management 

The risk management form has been integrated into the coach/assistant coach/volunteer registration process.  Please go to the registration website to
register as a coach, assistant coach and volunteer and as part of the registration process complete the Risk Management section. Coaches and Team Managers are responsible for ensuring all parent/team volunteers are aware of this requirement and informing the TBSC Risk Management officer about any non-compliance. Please, let's all do what we can to ensure a safe environment for our kids!

Code of Conduct

This form is required for all coaches and parent/team volunteers. Coaches and Team Managers - you are responsible for ensuring all parent/team volunteers, including yourselves, complete the form, and seeing to it that they get mailed to TBSC.

Medical Release Form

This is required and is available during the registration process

District Application

District 1 currently has an online registration form to complete by July 20th that requires a $60 application fee.  You can access this form by visiting the District 1 web site http://www.wsysad1.com/lpa/index.asp. 

Please note: Teams registering for district level of competition will need to complete the online form and send TBSC a check in the amount of $500.00 by June 30, 2010. This team fee is a different approach than the higher player fee from 2004. It includes the district application fee and costs associated with the additional games district teams play. Coaches and Team Managers - you are responsible for getting the team fee and application form to TBSC by June 30, 2010. Thanks!!!.

Scholarship Form - email our registrar (tbscregistrar@hotmail.com) about how to register with a reduced player fee.

Club Sponsorship Form - Please mail the form to the Terrace Brier Soccer Club, P.O. Box 423, Mountlake Terrace, WA 98043.

Coaches Evaluation Form - Please print, complete, and mail-in this form after the season. We appreciate your feedback! Thanks!!!
